We participate with most insurance plans.

Please call our office to see if we participate with your specific plan or call member services of the insurance plan.

Our office fees are based on national averages and are re-evaluated on a yearly basis. HIPPA regulations require us to copy your insurance card at every visit. Co-pays, past due balances, and your share of visit payments are expected at the time of visit. For your convenience, we accept cash, check, debit, or credit card. (We do not accept American Express). We will gladly work with you to establish a payment plan that meets both of our needs. However, if there is no response from you or your insurance company, your account will be turned over for collection by an agency and their fees will be added to your balance. If you have questions regarding your bill or balance due, it is your responsibility to notify us.
