We ask that you come 15 minutes early for your appointment in order to complete paperwork and the new patient check in process. Whenever possible, we mail out new patient packets which we encourage you to bring completed to your first appointment. You are also able to download these forms from this website if it is more convenient for you. Once you have an appointment scheduled, we encourage you to request a MyChart Patient Portal account. It is an excellent way to keep in touch with our office and be more involved with your healthcare.

Please be aware that we may not be able to immediately assume all of your prescription refills as it is important that we review your old medical records and get familiar with your treatment plan. You need to ensure adequate refills from your previous primary provider to avoid a lapse in therapy. In particular we do not prescribe controlled substances on a first office visit.
